Social Guest Policy

There is a flat fee of $150.00 to register your family for the Pre-CWAG reception and dinner. If you wish to bring a spouse and/or your children to the dinner, please continue to fill out the social guest fields below and click submit by July 13, 2012. After the registration is submitted you will be taken to a page with payment instructions.

Reporting Requirement

Federal law requires that DAGA report the name and address of all contributors that give an aggregate of $200 or more per calendar year to the organization.  If you (the attendee) or a third party is providing some or all of the cost of your or your guest's attendance at a DAGA event, please provide that individual or entity's name and mailing address, occupation and employer (if an individual), as well as the date and amount paid by that individual or entity for such attendance below. Note: social guest fees will be deposited into the DAGA Administrative account and will not be used as contributions in any federal, state or local election.
